- In an attempt to reproduce referred pain from trigger points, Hong and colleagues18 applied pressure over trigger points until their fingernails blanched (estimated to be 3–4 kilograms per square centimeter). They were able to reproduce the patient’s referred pain complaint more than 80 percent of the time. It usually takes a few seconds of sustained pressure against the trigger point to produce the referred pain, but occasionally it may take up to 10 seconds or longer.

- TMJ is estimated to account for as much as thirty billion dollars a year in lost productivity. Americans lose 550 million work days every year due to symptoms associated with TMJ, with facial pain and headache being the most common complaints
- Increased stress levels are believed to result in poor habits, including bruxism, clenching and even excessive gum chewing. These lead to muscular overuse, fatigue and spasm, and subsequently, pain.
- Phases of Treatment for TMJ myofascial pain:
- Phase one treatment is initiated upon diagnosis, and consists of educating the patient on muscle fatigue and spasm as the cause of pain and dysfunction. It helps to explain referred pain. The avoidance of clenching and grinding is emphasized, and a soft diet is instituted. Nonsteroidal antiinflammatory agents are prescribed, with or without a muscle relaxant. The most commonly used agents are valium and ibuprofen. One half of patients will obtain significant relief in 2-4 weeks.
- Phase II therapy is initiated if phase I treatment fails. Medications are continued, but a bite appliance (splint) is added. This helps prevent muscle overuse, including bruxism. The appliance is usually worn at night, but can also be worn during the day if necessary. Care should be taken to instruct the patient not to wear the appliance at all times, as the posterior teeth may become displaced. An additional 25% of patients will receive relief with this therapy. Once relief is obtained, the medications are discontinued.
- If phase II therapy alone fails, physical therapy of the muscle groups, including ultrasonic therapy, electrogalvanic stimulation or biofeedback are added. No one form of treatment is superior. Another 15% of patients will find relief within four weeks. If phase III therapy fails, psychological counseling is advised to identify stresses, and patients are referred to a TMJ center. TMJ centers employ a multidisciplinary approach, including psychological counseling and trigger point injections, for treatment.

- Do you know what the worst part is? That us hard working tax paying citizens are going to pay for her to both have this baby (no doubt with severe retardation if it does not miscarry due to the antenatal abuse) and to bring it up (appaulingly) to do the same... - In state after state, bleak statistics and grim anecdotes lead to the same diagnosis: America suffers from a serious, long-term shortage of child psychiatrists that is taking a toll on young people, their parents and their doctors.
- "There is no state where it is not a problem -- none," said Dr. Gregory Fritz, director of child psychiatry at Brown Medical School in Providence, Rhode Island. "We see it in the emergency ward every night, where problems have gotten out of hand over time due to lack of intervention, and progress to a point where a kid is suicidal or dangerous."
- Dr. Fritz complained about the escalating use of drugs for ADD and behavioral problems, estimated to be as many as 2.5 million children a year. Fritz said most are prescribed by pediatricians and other non-psychiatrists, saying "They're probably overused, and often without appropriate assessment and monitoring. If there were more child psychiatrists, they wouldn't be used as casually as they are."
